Advanced Knee Surgery for Dogs
See the Difference TPLO Surgery Can Make
"I cannot say enough wonderful things about Dr. Gholston and the entire MAH staff for the exceptional care they provided to our Sadie Lou during her recent TPLO surgery. We found out on a Monday that she had torn her CCL, and by Thursday she was already in surgery, which gave us tremendous peace of mind knowing she was receiving prompt, expert care.
Dr. Gholston's skill, knowledge, compassion, and genuine passion for veterinary medicine continue to give us complete confidence in trusting him with both our pets' routine care and highly specialized procedures like TPLO surgery. The follow-up care was equally outstanding—someone checked on Sadie Lou the very next day and again several days later to make sure her recovery was progressing well.
Dr. Gholston and his team make Sadie Lou feel like she is one of their own pets, and that level of kindness and dedication is truly priceless. We are beyond grateful for everyone at MAH and would recommend Dr. Gholston and his staff without hesitation to anyone looking for outstanding veterinary care."

What TPLO Surgery Involves
TPLO surgery stabilizes the knee (stifle joint) after a CCL rupture by altering the knee's biomechanics rather than replacing the ligament. By leveling the tibial plateau, the joint becomes more stable, pain is reduced, and further arthritis damage is prevented. This procedure is considered the gold standard for active dogs, offering the best chance of restoring mobility and quality of life.
Before surgery, dogs undergo a thorough pre-operative evaluation, including blood tests and imaging, to ensure they are healthy and ready for anesthesia. Digital X-rays are used to precisely plan the procedure, allowing the surgical team to correct the tibial plateau angle accurately and provide the best possible outcome for long-term joint stability.
Pre-surgical Preparation
- Precise digital X-rays to plan the operation
- Thorough pre-surgical assessments, including bloodwork and imaging
- Evaluation of the meniscus and surrounding joint structures
During the Procedure
- Cutting and rotating the tibial plateau to decrease its natural slope
- Repositioning the bone to a more level angle (around 5 degrees)
- Securing the new alignment with specialized titanium plates and screws
Post-operative Care
- X-rays confirm the corrected tibial angle and implant placement
- Incisions are bandaged for protection
- Dogs typically stay overnight for monitoring, pain control, and supportive care
- Discharge includes pain medication, antibiotics, and detailed recovery instructions
- With a newer pain control med called Nocita (time released 72-hour bupivacaine) most dogs can go home same day
Why TPLO is Recommended
- TPLOs can be done on complete or partial CCL tears
- Persistent lameness or joint instability
- Large breed dogs or significant knee degeneration
Post-surgery Recovery & Rehab
- 8–12 weeks of restricted activity for proper healing
- Structured rehabilitation, including controlled leash walks, physical therapy, and at-home exercises
- Follow-up visits to monitor healing and adjust recovery plans
